Two clinics use the same PDRN ampoule and the same pen. One reports steady texture gains by week four; the other reports "nothing dramatic". The ampoule is rarely the variable — the sequence is.
Step 1 — Prepare a truly clean canvas
Every residue is a barrier. Makeup, sunscreen and surface oil must come off completely before anything active touches the skin, because channels created by a pen will carry whatever is sitting on the surface with them.
Step 2 — Exfoliate to your protocol, not to a rule
Exfoliation is calibrated to the patient in front of you and their current skin condition, not to a fixed recipe. Over-prepared skin is more reactive, not more receptive.
Step 3 — Apply evenly, then stop
An even film across the treatment area is the goal. More product does not mean more delivery; delivery is the device's job.
Step 4 — Let the device do the work
Depth, speed and passes are adjusted to skin condition, sensitivity and the goal of the session. This is where clinical judgement outperforms any protocol sheet.

Step 5 — Close the barrier
Finish with a calming, barrier-supporting product and — for daytime treatments — broad-spectrum sunscreen. The session is not over when the pen goes down.
The 72 hours patients usually get wrong
- Gentle cleanser and a soothing, barrier-supporting moisturiser only
- Broad-spectrum SPF 30 or higher, every day without exception
- No scrubs, brushes or peeling products
- No retinoids, AHAs, BHAs or benzoyl peroxide
- No saunas, steam rooms or strenuous exercise immediately after
- No picking, rubbing or aggressive massage of the area
- Postpone waxing, laser and further resurfacing until the skin is back to baseline
Send this list home in writing. The most common cause of a disappointing four-week review is an enthusiastic patient reaching for their retinol on night two.
